Those who have any medical conditions should also bring any documentation that proves they have undergone treatment for those conditions, as well as medical records from the past 2 years.
Once your examination is complete, you should complete Form I-693 (Medical Examination Report) and submit it to the medical examiner. The medical examiner will use the results to determine whether you meet ICE medical standards and, if so, will determine your eligibility for green card issuance.

Police Clearance

When applying for an American visa, you will need to provide a police clearance from each of the countries where you have lived for at least six months. If you are applying for a green card, you will also need to provide a police clearance from each country where you have lived since your 16th birthday.
If you do not have a police clearance certificate, you can find one on the U.S. Department of State’s website. Just search for the country or region where you live and look under “Police Record.”
In some cases, it is possible to submit a certified copy of a police clearance certificate that was obtained in your home country instead. However, you must make sure that this certificate is dated and has been issued within the last two years.
Similarly, you cannot submit an American visa application if you have been found guilty of any crimes or if you are convicted of any crime that requires a jail sentence. This is because a criminal record can have negative impacts on your application and could even prevent you from receiving a visa to enter the United States.
Financial Documents
To apply for an American visa, you must provide a number of financial documents. The requirements depend largely on the reason for your trip, but in general you must have proof of financial stability. This can be through a bank statement, income tax return or other financial document.
If you’re a student or self-employed, you must also provide a letter from your employer or an official reference from your education institution. This letter should indicate your status and expected graduation date.
For children travelling with a parent, a notarized letter from the parent stating that they give permission for the child to travel and copy of the birth certificate should be provided. For parents with shared custody of their children, a notarized letter from the other parent giving permission for the child to travel should be provided.
There are also some extra documents that are required if you’re traveling for business purposes. These include a bank statement, income tax return and a letter from your employer confirming that you’re in good standing.
You should make sure that all of the financial documents you submit are in a format that is recognized by the Consulate. Some of these documents can be printed or obtained online, but other types must be originals.
The photos you use for the visa application must be in the passport format and be taken in front of a light background. They should be brightly coloured and show full face features. You must also submit two recent (less than three months old) photographs, one of which should be stapled to the application form.
